Depending on exposure, his lungs might burn out. He also stands to face radiation exposure if he's outside too long. A lot of this depends on time more than anything.
He would be able to quickly tell that the air outside is legitimately awful, though. Even if it's a simulation, whatever being done will dramatically impact his ability to breathe and function. So, good luck Jay!
